Plan, direct, and coordinate medical services for special needs children at May Institute. Review patient documentation to determine Medicare eligibility and review Medicare Plan of Care (485's) on start of care and for re-certification. Review transfer patients for possible adverse events, analyze trends and interventions that may prevent future uses of emergent care. Review discharges with focus on outcomes, appropriateness of discharge plan, and follow-up time points and 485's.
Must maintain current knowledge of state and federal regulations, industry and CMS standards and guidelines pertaining to home health care.
Requirements:
Bachelor's in Nursing and MA Nursing License. Employer will accept any suitable combination of education, training or experience.
